*An entry e-mail/card required by each individual applicant.  Although partents and family members who wish to accompany their children is not the case, please note that they also need to pay workshop fee for entry & to declare as such in the application.
*Place allocation will be made by a draw if we receive more applications than we can accomodate. The result will be informed to all applicants by the end December.
*We will only use your personal information for administrative purposes of "Organ for All" program.
